Life story — central to person-centered care

What is meant by a "life story"?

A life story describes a person's life history, values and preferences, and is used by care staff to provide person-centered care — especially important for people with dementia who themselves find it difficult to convey who they are.

So far, most of it is done on paper

Today the life story is usually documented on a paper form that is handed over to the home care service or nursing home upon move-in — often during an already stressful period of life.

The National Board of Health and Welfare wants everyone to have a life story

According to the National Board of Health and Welfare's national guidelines for dementia care (2018), over 98 percent of people with dementia should have a documented life story. However, in the quality register SveDem, only 76 percent of registered individuals in nursing homes had a life story as a basis in 2018, and in home care as many as 42 percent of providers in a 2017 survey said they never obtain the life story.

Digitalize with MinMemoria

MinMemoria from VilMer makes the life story digital and accessible to home care, day activities and nursing homes. Family members can contribute pictures and text on an ongoing basis, and the information becomes a living resource rather than a one-time document.

5 reasons to digitalize the life story

  1. A living, constantly updated story.
  2. More family members can be involved digitally and securely.
  3. Family members get better insight into daily life.
  4. Pictures spark memories and reduce anxiety.
  5. New staff and substitutes get to know the resident faster.
