What do the letters stand for?

H — Accountable: responsible for ensuring the task is completed, the final decision-maker.
U — Responsible: the person or people who carry out the task.
K — Consulted: consulted for input but doesn't make decisions (two-way communication).
I — Informed: needs information but isn't directly involved (one-way communication).

How to create a HUKI matrix

Identify all activities and decisions.
List everyone involved and their roles.
Define who does what — map activity against role.
Propose the matrix, get feedback and anchor it.
